'Human error' exposes patients' numbers

The Computerworld (US online) story, "'Human error' exposes patients' Social Security numbers" has been clarified to include additional information about a security breach at the Worcester Telegram & Gazette newspaper. The seventh paragraph has been rewritten and now reads:

In the Globe incident, confidential information belonging to more than 200,000 subscribers was inadvertently exposed when the Worcester Telegram & Gazette, a sister publication in Worcester, Massachusetts, reused paper containing their names, credit card numbers and bank account information to print routing labels that were attached to bundles of newspapers.
